No, not exactly. It's a bit unclear given the episode and the one before, but careful reading of them makes it seem as though this is what happened.
Ranma fell into the spring of drowned girl, then goes to attack Genma, knocking him into the spring of drowned sex demoness. In the process, Ranma then fell into the spring of fraternal twins, but was simultaneously splashed by the water from the spring of drowned sex demoness.
Thus, when Ranma is hit by cold water he becomes a sex demoness - but has been brainwashed by Ranko into believing he's cursed to turn into a girl.
But yeah, looking back over that episode I can see why it might be confusing.
